I understand and acknowledge that I am participating in an online fitness and/or nutrition coaching program offered by Fittivity. The program objectives and procedures are designed to help me achieve my personal fitness and nutrition goals, through personalized coaching and guidance from a certified fitness and nutrition coach.
Description of Potential Risks
I understand that there are potential risks associated with any exercise or nutrition program, including but not limited to, physical injury or illness. I acknowledge that I am participating in this program voluntarily and at my own risk. I also understand that it is my responsibility to consult with a medical professional before beginning any exercise or nutrition program, to ensure that I am in good physical and mental health. I assume full responsibilities for any injuries or damages which may occur during and/or after training.
Description of Potential Benefits
I understand that participation in this program may lead to numerous potential benefits, including but not limited to, improved physical health and fitness, improved nutritional habits, and increased confidence and self-esteem.
Confidentiality
I understand that any information shared with my coach during the course of this program will be kept confidential and will not be shared with any third parties without my written consent. I also understand that my coach may use de-identified data for research purposes, but my personal information will not be shared.
I acknowledge and agree that my coach may use anonymized and de-identified information about my fitness progress, including progress photos, body weight, and trends, for the purposes of social media promotion and marketing. These materials will not contain any personally identifiable information or show my face. This may include general before and after transformations or statistical data demonstrating client progress.
I understand that if I want to rescind this consent at any time, I will notify my coach in writing and all efforts will be made to promptly remove the previously shared information from the coach's social media platforms, understanding that once information has been shared online, total removal cannot be fully guaranteed due to potential sharing, copying, or saving of information by third parties.
